Referral forms for prescribing practitioners
To fill a prescription, patients must provide an original prescription and cannot fax it from home. Faxed prescriptions will only be accepted if sent directly from the medical provider who issued the prescription. We cannot accept faxed prescriptions from patients or third parties due to legal restrictions. Please visit your doctor or prescribing practitioner in-person to obtain the original written prescription to bring to our pharmacy. This allows us to safely dispense your medications under the appropriate oversight.
